San Diego Hospice hosts Free Workshop Advance Care Planning

San Diego Hospice and The Institute for Palliative Medicine invites the community to a free workshop "A Roadmap to Health Care Decision Making" on Saturday, June 2 from 9:00 a.m. to 12:00 noon. Family members, caregivers and anyone in the community impacted by chronic or serious illness are encouraged to attend and learn how to ensure that your healthcare wishes are met, especially when you are unable to advocate for yourself.

• Advance Directives
• POLST
• Durable Power of Attorney for Healthcare
• Getting your legal affairs in order
• Q&A Panel with Presenters

Registration, continental breakfast and resource tables will begin at 9:00 a.m. and the program will start promptly at 9:20 a.m.
