Family members\, caregivers and anyone in the community impacted by chronic or serious illness are encouraged to attend and learn how to ensure that your healthcare wishes are met\, especially when you are unable to advocate for yourself.\n\n	Medical and Legal Experts will discuss\n\n	\n		Advance Directives\n	\n		POLST\n	\n		Durable Power of Attorney for Healthcare\n	\n		Getting your legal affairs in order\n	\n		Q&A Panel with Presenters\n\n\n	Registration\, continental breakfast and resource tables will begin at 9:00 a.m. and the program will start promptly at 9:20 a.m.\n\n	Registration is required for this free workshop\; seating is limited.
